Position Summary: Responsible for all facets of program operation in a 45 bed residential drug and alcohol facility during assigned shifts, working directly with addicted mothers and their children.

Certain positions require health assessment upon hire and every year thereafter. Experience working in Windows XP, Internet Explorer and Microsoft Office. 13) Maintains safety, security, cleanliness of the unit, vehicle, and equipment and assures proper use of supplies. Conducts fire drills.
14) Operates a motor vehicle to transport clients to appointments.

Working Conditions: Position is situated in a 45-bed inpatient drug and alcohol treatment facility. May require visits to homes, schools, and/or high-risk environments. Exposure to clients with substance abuse, mental illness, and/or communicable diseases may occur. There may be a loud noise level at the facility due to babies’ and children’s activities.
Position Summary: The Care Manager is primarily responsible for coordinating support services to participants in the OPTIONS program along with their families/caregivers. The position major functions involve assessment, resource counseling and support, service
planning, service initiation and monitoring supports for the participants.
Position Qualifications: A Bachelor’s Degree in Social Work or a Social Service related field and at least two years of relevant experience in related services or case management. A minimum of two years experience and knowledge working with the older population is required. The Care Manager must possess a valid PA Driver’s License and access to a vehicle. Note the hours can vary to include working evenings and weekends as needed.
Primary Responsibilities/Essential Functions:
• The Care Manager will complete face to face visit and initial assessment packet within 2-4 business days after referral. Of these assessments, 95% will document a correct level of care determination and 100% of substantiating documentation.
• 100% of CM participants will have at least one referral to community resources quarterly.
• The Care Managers will produce a participant driven, strengths-based plan for each consumer that includes participation from family, caregivers, and any other person designated by the consumer.
• The Care Managers will initiate referral to a service provider within 2 business days from date of determination and choice by participant.
• The Care Manager will complete monthly contact by phone or face to face with each participant they support on their assigned caseload. If increased monitoring is necessary due to regulations, this will be completed as required.
• All documentation will be completed in SAMS within 24 hours of contact with a participant.
• For FCSP participants, the Care Managers will contact 100% of participants each month via phone or e-mail. The Care Managers will provide each participant a quarterly educational/training opportunity to support their needs.
• Attend all meetings, supervisions and trainings as required.
Part-time - 4 hours per week to start. $12.00/hr - $13.00/hr.
Position Summary: Community Inclusion staff provides one-on-one interventions to an adult autism spectrum participant in the home and other various community locations. The staff assists the adult participant by providing on-site modeling of desired behavior, behavior support, intensive behavior episode intervention, training, cueing, and/or supervision in order to help the participant increase their capacity to perform activities of community living and instrumental activities of daily living. The staff also assists participants in acquiring, retaining, and improving communication, socialization, self-direction, self-help, and adaptive skills necessary to reside in the community. The staff will help the individual reduce their need for direct personal assistance by improving the participant’s capacity to perform these tasks independently.
Position Qualification: Valid driver’s license. Valid auto liability insurance if assignments
include driving own vehicle.
Required:
A. Minimally a High School Diploma and two years of relevant experience working with individuals with Autism Spectrum Disorders, or a Bachelors Degree.
B. Expected to complete all of the required training upon hire developed by the Bureau of Autism Services for adults
with autism spectrum disorder.
C. Act 33 clearance; Act 34 clearance; and FBI clearance.
D. Health assessment that indicates “free from communicable disease and able to work with at risk population” upon hire and annually thereafter.
E. Schedule may require working evenings, weekends, holidays, and overtime as needed and/or as directed.

Working Conditions: This position provides “Field Based” services. As such, staff travel to various locations throughout Allegheny County to provide services. Most frequently these services are provided in the participant’s home, libraries, stores, volunteer activities and other community locations. Position Summary: This position provides support by entering and posting data necessary for billing, reporting, and statistical purposes in various internal and external database systems. This position will also review and analyze data through HCSIS for quality assurance purposes.

3. Review random sample of SC service notes to ensure compliance with TSM regulations and report findings to SC Program Managers.

Position Summary: The Program Manager provides supervision and oversight to the OPTIONS Care Management program. This position works with the departmental leadership in developing, interpreting and leading the implementation of policies, strategies and outcomes along with the supervision of the program’s staff.
Position Qualifications: A Bachelor’s Degree in Social Work or a related field and at least two years of Care Management experience. The CM Program Manager must have a minimum of two years experience and knowledge in working with the older population. The preferred candidate will have at least two years supervisory experience and experience in quality improvement and outcome measures.
Primary Responsibilities/Essential Functions:
• Oversee the team of Care Managers to ensure that compliance and regulatory standards are being met.
• Provide training and clinical conferences to Care Managers in the delivery of services to participants and caregivers. Conferences with staff occur one time per week and team meetings one time per month. Conferences are documented and signed by CM and Program Manager.
• Review, approve, and sign-off on case records and service plans for all participants on their team
• Assess the skills of the Care Managers on their team for training and education needs. Identify overall areas of training and secure appropriate resources to ensure training and growth occurs with staff.
• Interview, hire and evaluate Care Managers and assure approved participant/staffing levels are maintained. Evaluations occur at initial six months and annually thereafter.
